Effects of Heart Rate Variability Biofeedback in Patients With Acute Ischemic Stroke: A Randomized Controlled Trial
Abstract Background: Autonomic dysfunction, cognitive impairment, and psychological distress are associated with poorer prognosis in patients with acute ischemic stroke (AIS). Heart rate variability (HRV) biofeedback (BF) improves autonomic dysfunction, cognitive impairment, and psychological distress in other patient populations, but its effect in patients with AIS is still unclear. Objective: This study investigated the effects of an HRVBF intervention on autonomic function, cognitive impairment, and psychological distress in patients with AIS. Method: In this randomized, controlled, single-blind trial, patients with AIS were randomly assigned to the experimental or control group. The experimental group received four HRVBF training sessions. The control group received usual care. Repeated measures of HRV, mini-mental status examination (MMSE), and Hospital Anxiety and Depression Scales (HADS) were collected prior to and at 1 and 3 months postintervention. Results: A total of 35 patients completed the study (19 experimental, 16 control). HRV and HADS significantly improved in the experimental group (p < .001) but not in the control group. Likewise, only the experimental group showed significant improvements in HRV, MMSE, and HADS over time (p < .05). Conclusion: HRVBF is a promising intervention for improving autonomic function, cognitive impairment, and psychological distress in patients with AIS. More studies of HRVBF interventions are needed to further optimize the effects of HRVBF on autonomic, cognitive, and psychological function in patients with AIS. 要旨
背景:リンパ浮腫は慢性、進行性で、軟部組織の高たんぱく性の浮腫です。リンパ浮腫検知に、患側の細胞外液の測定がよく行われています。しかしながら、リンパ浮腫の皮膚変化や早期の組織変化の定量化はあまり行われていません。
方法と結果:91名のリンパ浮腫患者を評価しました。皮膚水分蒸散量(TEWL: transepidermal water loss),皮膚の固さ(SF: skin stiffness)、水分含有量(PWC:percentage water content)をあらかじめ決めていた皮膚の5箇所で評価しました。TEWL、SF、PWCの値は、対照群(健側)と比較してリンパ浮腫皮膚で有意に増加し、患部皮膚の機能と肌理が損なわれていることを示しました。PWC比とSF比はいずれもリンパ浮腫のステージと強い相関関係がありました。組織液を評価する機器と高い相関が認められました。
結論: 皮膚パラメータ評価は、慢性リンパ腫皮膚の機能的および構造的変化に関する新たな情報をもたらしました。皮膚特性の変化を定量化することは、慢性リンパ腫の診断と評価を補完する重要な手段となるかもしれません。
